Position Overview:
This position will be responsible for intake and case management for elder client case load, including needs assessment, service plan development, service authorization, and monitoring service needs of clients.
Qualifications:
Bachelor’s degree with prior experience in Human Services, preferably with Elders. Valid Massachusetts driver’s license, clean driving record and reliable vehicle for the ability to make home visits. Ability to actively participate in self-directed team environment with excellent communication and documentation skills required, Spanish bilingual preferred.
Hours:
35 hours per week.
