The Caseworker investigates and develops service plans to work with seniors at risk of abuse, neglect and self-neglect. Daily responsibilities include home visits, crisis response and assessment, service planning and work with family and medical systems to support elders living in the community.
Master's degree in Human Services with 1 year related experience or Bachelor's Degree in Human Services with 2 years experience required. Must have a driver's license and reliable transportation. Position may include some limited on call responsibilities. Clinical experience with high risk consumers living in the community, prior work with mental health and substance abuse and strong advocacy skills a plus.
This position is a part time, 25 hour position with the potential for full time employment in the future.
